We are glad that you have decided to join the NC-ACTS! AmeriCorps program. Below you will find program requirements, links to important information, and general policies. If your question is not answered here, please contact your Campus Coordinator. Program Requirements:
These are the program requirements each member must fulfill to remain in and complete the program. You can download all relevant forms in the Forms section of this website.
1. Complete enrollment paperwork
- Corporation for National and Community Service Enrollment Form
This form asks for general information about you and requires your signature only.
- NC-ACTS! Member Agreement
This is a contract between you and the NC-ACTS! program and outlines all the requirements and expectations involved with the program. You and your Campus Coordinator must sign the agreement.
- Member Eligibility Verification Form & Copy of ID
This form will be signed by your Campus Coordinator after you show them your original Birth Certificate or Passport as proof of citizenship. They will send this form with a COPY of the document you show for proof of citizenship. A Driver’s License is not an acceptable form of ID.
- Service Site Agreement
This form provides a brief description of the service you and your Site Supervisor have agreed that you will complete during your term of service. This form requires you and your Site Coordinator to sign.
- Site Supervisor Agreement
This form acts as a contract between your Site Supervisor and the NC-ACTS! program and requires the signatures of your Site Supervisor and Campus Coordinator.
- Enrollment Checklist
We provide a checklist that serves as guide to ensure that you are completing and returning all the necessary paperwork to properly enroll in the program. You will not be enrolled until all these forms are received and correctly signed by the necessary parties.
The following form is only required in specific cases.
- Criminal Record Check Verification Form
As required by the Corporation for National and Community Service, all NC-ACTS! members who accrue hours during which they have regular contact alone with children or other vulnerable populations must submit a completed Criminal Record Check Verification Form along with their Enrollment Paperwork.

2. Attend any orientations or trainings that your campus will host
Your campus coordinator will either schedule a group orientation or one-on-one meeting with you to discuss the program requirements and expectations before you enroll in the program.
3. Serve your hours every week!
As the NC-ACTS! program is developed to work within the academic calendar, you should serve hours every week to ensure completion of the program. On average we recommend 10 hours per week.
4. Complete a monthly Time Log
In order for your hours to be registered, you must submit your time log to your Campus Coordinator at the end of each month. The form MUST be signed by YOU and YOUR SITE SUPERVISOR. Please submit hours for one month PER time log. If you serve at multiple sites, you will submit one time log per month/per site. Please make every effort to submit logs from multiple sites all together.
5. Participate in at least (2) Reflection Sessions
Every NC-ACTS! member is required to complete at least two reflection sessions. These sessions will be scheduled and designed by your Campus Coordinator. These are intended to deepen your service experience by allowing you the opportunity to process and discuss your experiences with other NC-ACTS! students. Your Campus Coordinator will take attendance at these sessions to verify your participation. Your Campus Coordinator will sign the Exit Checklist to verify completion of both sessions.
6. Complete the NC-ACTS! online Exit Survey
Every NC-ACTS! student must complete an online survey before exiting the program. NC-ACTS! program staff will e-mail the link to the survey once you near or exceed 200 hours of service. Please save the link. The subject header will read "NC-ACTS! End of Service Survey." Sometimes this link goes directly into your spam folder so be sure to check it regularly upon exceeding 200 hours.
7. Complete your Exit paperwork
- Corporation for National and Community Service Exit Form
This form asks for your contact information and whether you wish to receive additional information from AmeriCorps. Your signature is required.
- Site Supervisor Evaluation
Your Site Supervisor must complete an evaluation and assessment before you are exited from the program. They may give the evaluation to you to give to your Campus Coordinator or they may mail it directly to your Campus Coordinator or the NC Campus Compact office.
- Exit Checklist
We provide a checklist that serves as guide to ensure that you are completing and returning all the necessary paperwork to properly exit. You will not be exited from the program until all these forms are received and correctly signed by the necessary parties.

Reasonable Accommodation
The NC-ACTS! program will provide reasonable accommodation to the known mental or physical disabilities of otherwise qualified members.
Accommodations that impose an undue financial or administrative burden on the operation of the program or fundamentally alter its nature are not reasonable accommodations.
